When a quiz is load for a student for the first time, there is PHP warnings such as in the attached example >> https://www.dropbox.com/s/m88j067e3mr1rwl/S__29212773.jpg?dl=0
- I had made sure php.ini has all error/warning disabled.

Please update your themes and plugin to latest. we have already fixed many of this type of issue. current version is 3.9.5
