Apply for Course Form Addon

Hi. I have added the wplms "Apply for Course Form" Addon but it does not work properly. When students click on "Apply for Course" button it is suppose to display the form according to the Addon write up. Instead it subscribes and enroles the student to the course. I need students to apply for the course by first completing a form. After reading comments and watching how to videos. I need to clarify what I need. For certain courses when the student clicks on the "Apply for course" button they need to be redirected to the form above. That is what I understood the addon did.

@RMA, As the name suggests, this addon is for those who want this feature, that users need to first apply and then only after administrators or instructors approval the form will be approved. And the users will be enrolled in the course. You have to fill the form and upload the required documents, then click on the button to apply for the course.
