Vibe course module plugin : 1.9.8.4 wplms-assignments : 1.9.8.2 vibe customtypes : 1.9.8.1 Please also make sure that you have not renamed the theme folder while installing it . Please also try deactivating the third party plugins and check again .Also, in our experience, uploading assignments won't work if you have your WP installation anywhere other than the root folder. If you have WP installed on a subfolder of the root, like, let's say: yoursite.com/wpinstallation, then the upload feature won't work.
