problem in quiz answers

hello in quiz ,, when student give wrong or right answers it always show wrong sign what is the problem

Hello, Please first make sure quiz connected with course and course connected with quiz also deactivate all third party plugin as well, Update all thing according to latest theme version, Thanks,
