hi yes because the first twos are default fields so if you want to keep them then we can change the styling with the custom CSS let us know

It works for the pc . But when I am checking it from my mobile phone it is showing like this https://ibb.co/YPxYhdy. Please check from a real android mobile not the Google Chrome inspector tool. From the Google Chrome inspector tool, it is ok.
