The image does not appear to the student when I insert an image through the Amazon S3.
It doesn’t actually appear to the instructor.

Yes this is an issue right now s3 images cannot be assigned as course featured images .
You can upload the iamges locally and use them since featured images are not supported from third party media library right now in wordpress itself .
