Add Media not working in LMS

Need to create new tasks, new classes but after update "Add Media" and other features don't work. Anyone has a clue about a solution? I saw that I have a JavaScript error: "Object doesn't support property or method 'chosen'" Not sure if this could help to understand the question. Thanks in advance.

Make sure that your wplms theme , modern child theme and all its plugins are updated to latest version . Re-save permalinks and clear any cache in site and browser after update . Also try disabling the third party plugins for this . If above does not  helps  then please share site url and admin credentials to check the issue at your end .

I received a similar error, when I click on add media nothing happens. I am attaching a file showing error and corresponding browser console of firefox... Please Help !!!!
