Error when check the Quiz Result

Well i checked it ,There is something really strange .It seems like the question are getting deleted from user's meta table when user submits the quiz . I tried to find where this is happening but failed to find it .Will check again provide me some time for this

Yes, because when the exam is retaked the problem is solved... and many user have spend time in exam and then lost their results, they have to retake the exam and spend more time. It is awful!

Well yes you might be right at the very first time when user takes exam it shows the error . I guess there might be something wrong with that quiz itself . CAn you please share me your whole php server error log . (ask webhost to find it ) . Also try creating a new quiz and check if this issue does appears with that also ?
