Evaluate Assignments – Unregistered Action

I am unable to evaluate assignments in my course. From the front end, it isn't showing assignments that have been submitted.

• The assignment plugin is activated.
• Students can answer and submit, but I cannot evaluate
• I see 0 assignments to evaluate, but I see 5 assignments that I have created
• When I look at course activity it shows an unregistered action. See image attached.
