HTML Codes are no longer working on my site

All the HTML codes are not working anymore on my website. See an example on contact form. Hi... As mentioned in another topic of yours, it seems like the shortcode plugin is deactivated in your website, otherwise there is no reason of showing the shortcodes on website.
