media library issue / not inserting

You guys have a great product and I am always telling others to use it. Currently I have just started a project with WPLMS and am having a problem that I can not diagnose.</p> <p style="box-sizing: border-box; margin: 0px 0px 1em; padding: 0px; color: #666666; font-family: 'Helvetica Neue', Helvetica, Arial, sans-serif; font-size: 14px; line-height: 1.5; background-color: #fafafa;">When I click from your options panel to upload our logo or fav it acts like it did it but nothing shows up and upon save and updating there is no change and the options panel says “upload image” as if i did nothing.</p> <p style="box-sizing: border-box; margin: 0px 0px 1em; padding: 0px; color: #666666; font-family: 'Helvetica Neue', Helvetica, Arial, sans-serif; font-size: 14px; line-height: 1.5; background-color: #fafafa;">Same issue inside Visual Composer when trying to insert images. If i delete something or click remove image it will. but then nothing happens when I go to add the new photo. It brings up a weird popup for choosing the image that is different than the normal wp media pop up, i click show, click use this image. then… nothing.
