Quiz is showing result as wrong answer when all the answers are marked correctly

Hello, After creating quiz, when we take the quiz and mark all the answers correctly, after submitting, it is showing all the answers as incorrect. Kindly see the below link and screenshot : http://pharmalearners.com/members/amarcj/course/course-results/?action=1113 http://pharmalearners.com/quiz/tablets-capsules-micro-encapsulation/ Refer the second quize "Tablets, Capsules & Micro-encapsulation" Quiz results are showing all answers are incorrect, however i have answered the all correct options.
