Some Fields are not showing in registration form and vibe form

The issue of some fields not showing has happened to two forms, one is a registration for instructors and a vibe form shortcode used

1. Instructor registration

Additional issues

When users register on the website using email, they do not receive activation mail. I have confirmed that the website is sending emails, by trying to send a reset password and the user received it. Only users who register using Google email authentication receive email.
