We checked your site and debugged .These are my findings :
The script that we are was showing the its execution to be working fine in console but still the image is not appearing .
We are now trying not to use that script and depend solely on browser print function.
We will test this and write back to you soon .

Ok I checked your certificate image and something in it was causing issue .
I downloaded it and compressed it online and then rename and upload it again to your site and it worked fine .

Sorry for adding in between as this is a very long topic and I may have missed some key parts :
I suggest followings settings for W3TC :
1. Stop page caching for Logged in users.
2. Try adding regex code to stop caching the user profile altogether.
