I would first thank you and congratulate you for the huge work you've done on the appointments addon !
I prefer to open this ticket rather than use email chat.
I installed the beta version and setting it up. Everything seems working fine, except when we try to book from an instructor profile already enabled, we get an HTTP 404 error.
If this might help, I would like to notice we are using the Cloudflare full strict https and a plugin called Hide My WP.
Your help will very appreciated !

Go to wp-admin > Settings > Permalinks > check the slug (for course-booking is set). Then save settings

If you can't give me access of the plugin editor then please check the wplms customizer plugin. Is there any custom code you added in the file.

I checked the two probabilities you have suggested previously.
"1) The permalinks are mismatched, in this case you need to go to wp-admin->settings->permalinks, and re-save your permalinks."
-> There is no "membres" anymore. There is only "members". And this is not the source of the issue.
2) There is some custom code added which is interfering with the functionality, so try checking your customizations (if any).------> check in the wplms-customizer.php that did you added any custom code or not
-> I deactivated the file wplms-customizer.php and checked again, all customizations are gone as normally, but the button book instructor is not working neither.
I hope we are helping you to eliminate the wrong paths until finding the source of the issue.
