we have a strange situation. We have around 400 students in a course and lately, when we mark an Assignment, at times the wrong student receives the grade!
Yes, basically we grade Students A but it is Student B who gets the email, message and the grade added to their Results page...
This is odd. What could be causing this?

This is very weird and it's not possible that some other user is receiving the grade of some other user's assignment.
Please share the url and the admin credentials to check this.

I read through the reply, we have made the fix of those custom fields and since then have been creating new assignments rather than copying them.
Now we are still experiencing the same issue stated at the start of this thread.
Students A submits an assignment and Student B receives the results via email and on-site notification.
Do not use the credentials in the previous post to connect, I will give you the credentials in a private reply to login and try and fix the issues. note that I have disabled WooCommerce Subscriptions, Emails and a few other things as it is a staging site. You may disable plugins and play with it as you like, please just report back what you have done by the end of it.

please note that this issue is (luckily) only presenting itself rarely. That is why we provided you with the names of students who got the wrong feedback. Hopefully you can did in logs to find a reason.
Simply submitting an Assignment will not replicate the issue.
Even when my team corrects your Assignment, the issue may not present itself.
You will probably receive our feedback and notification with your grade.
The problem is that, at least once a week, a student receives an email and a website notification, for an Assignment they have not submitted, and even from courses they do not own!!

There is no such code that it will work correctly sometimes and sometimes it will not. The code executes the same everytime and I am not sure what's causing the issue.
I need a use case or steps to replicate the issue then only I can debug the code from where the issue arises and then fix it.
Yes I am aware how code works, I'm a programmer.
But there is a bug somewhere in your code or a conflict with other code that needs fixing.
Unfortunately we have not identified the steps to replicate the issues as it seems to only happen to specific people. But I will give you a few more examples and you might be able to find that all the people have some meta data that is conflicting something.
The steps that would have happened is what I have already mentioned:
- Student Submits Assignment
- Tutor Marks Assignment
- Different Student receives results and emails etc
