I created a quiz in version 3.xx, and when I change it, 4.0 does not display changes, even a list of questions.
In the video below - the quiz contained 15 questions, I deleted 6, left 9. But the quiz still shows all 15 questions. I tried to regenerate the service worker, but it did not help.

you have clicked in the save element
you need to click on this button:

and then publish the course then it will save the changes you made

and the credentials with I can check the same
and also I request you to check with the new user as the changes will not reflect the user who is already enrolled in the course

it will not reflect on the retake quiz you need to remove the student first and then add again
then it will show the modified quiz
Thank you for reply. I removed user and added him to course but it didn't help - the quiz is still displayed as unchanged for this user. Any other advice?
