APP not being created in spite of all settings are proper

Hi

you can see my early chats and tickets 
i am stuck exactly in the point of app..... 

100s of times I have tried to communicate you. From this point onwards I will be in dark.

please set my app at least this time. all that needed Is after generating service worker it is not generating app. it shows some manifestation error. 512x512 icon seems to be not being connected. but in website I have added icons twice in fact right now also it is present still not able to generate app..

please fix this at least this time
