Course satisfaction questionnaire

Hi, I have to insert at the end of the course a course satisfaction questionnaire. Each participant has to submit it. There are a few questions, students can insert their comments or they can indicate the grade they appreciated something in the course (example: a. excellent; b. good etc.). It would be perfect if they can express their evaluation anonymously. I created the questionnaire using quiz model, but the result is not so good (for example the button says “Start quiz” when the student already completed the course). Can you suggest me a better solution?

@studiomedicopavesi_0412, But you can add as many quizzes as you want in a course. Do not include the quiz in the course and add survey type questions Or you can attach it as an assignment at the end of the unit
