Dynamic Quiz questions are not displaying (only possible answers)

Hi, I've set up a dynamic quiz and the question title isn't showing up (I do get all the possible answers and marks, but no question to actually answer), on which file can I check this out? or any ideas on how to fix it? Thanks.
