Help setup the membership level page

I need some help setting up the page where members can choose membership. I have created a couple of membership levels, but they only display as a list without any descriptions.

• I use the default theme

I want it to diplay like this: http://themes.vibethemes.com/wplms/skins/oneinstructor/membership-account/membership-levels/ I cannot figure out how to get it to display like this. I know it have something to do with the child theme, but can not find how to set it up. Thank you for your help

Well feature at this is a custom built shortcode for pmpro memberships level . you can create pricing table with the help of third party plugin pricing table plugins .
