insert new unit from backend

Why can't I insert a new lesson from backend on version 4, as i used to in the previous version? https://prnt.sc/10rz41e Notice the lesson exists already, called APOCOPE on image on the right. I try to insert this lesson in a course that suggest this lesson doesnt exist, on the left. I have tried many other lessons, in many different courses: always the same result.
