Questions are not getting created using .csv format (tried frontend/backend both

Hi, I am trying to upload questions using .csv format in bulk. i tried front end as well as back end option, it's not working. Tried to create question using front end manually, that is also not working as it keeps on circulation on create question button. Tried the following video https://www.youtube.com/watch?v=gut995vlcec&t=4s but my front end screen shows differnt option than this video when i click on download sample button? Can you advise what to do? using demo 2 and tried all above options with demo 5 as well. Thx Sara

The issue could be related to table prefix, It is possible that both database table prefixes are different which restricts the export
