There is any way to create a admission form in frontend?

Hi Support, I want to create a custom admission form in front end and user will fill the form and form entries should be list in the admin panel. I have created the custom HTML form but I need to view the entries in the admin panel as well. Also form will submit after complete payment. I will use our local payment gate way like PayuMoney for payment. Also, there is functionality to user will able to upload their documents like image and Id proof.
