We have to debug this strange issue but I believe that should resolve once your login and logout .
Yes, it seems that it is solved by logging in again, but will this happen to users who register for the first time? It seems that the only place that fails is the front, inside VibeBP it works correctly.

The error is something else ,I misunderstood this please try :
goto wp-admin-> settings -> discussion -> Avatar Display-> enable this and save settings . then again check this up .If still does not resolves then please share your site url and admin creds in private reply ,
